Abstract: History is a mirror that can reflect the past, examine the present and predict the future. This motto will be used to tell the story of an engineer who made a career in orthopaedic research & patient care. Engineers, trained differently as opposed to the physicians or medical scientists, are rare to become a full-fledged faculty and a staff member in a clinical department. It is even harder for them to be accepted as equals among the clinical specialists. The legendary story of the ancient Greek King Ulysses is used to illustrate the extraordinary hardships and rewards an engineer had experienced in an alien field. It is easier to accept the new and abandon the old, but King Ulysses never lost his determination to fulfill his primary obligations. Thus, he was able to overcome the insurmountable challenges under the divine protection of the Goddess Minerva. In a similar manner, this engineer had the confidence and support from his colleagues and the fortune of being at the right time and the right place to pursue his undeterred aims under harsh conditions. All warriors will age and fade away, but the new breed shall follow the path and reinstate the glorious days they once established, kept and prevailed.

INTRODUCTION: Orthotic devices have been used for more than 2,500 years since the ancient Egyptian era and their improved design, fabrication and application through time made them an important and well-received medical and health maintenance apparatus for many decades. However, this category of medical devices and its therapeutic value have been grossly ignored in the health care field recently. Consequentially, the technical advances related to their design, manufacturing and clinical utilizations have been lagged far behind other devices and techniques mainly due to the rather subtle socio-economical factors and the medical hierarchical order, and thus, lowering the orthotic devices’ importance and their deserved clinical attention. Thanks to only a very few forward-thinking designers and manufacturing firms who are willing to devote their experiences and resources to develop the new generation of orthoses to improve their efficacy, comfort and ease in application to benefit a large patient population. The expanded use of these devices in the field of trauma, injury prevention and rehab will undoubtedly stimulate their future development to fully explore other possibilities. This presentation introduces the new concepts of orthotic device design, manufacturing, and application which will drastically reduce health care cost without short-changing its quality. It is hoped that the advances in orthotic technology gained in the spine arena will stimulate the same changes in other anatomical regions which will impact a large patient population involving trauma, occupational hazards, age-related degeneration and other musculoskeletal diseases and deformity.
